Gift Certificate Order Form

Gift certificates may be ordered in any amount. There are no fees for using GPH gift certificates and GPH gift certificates never expire! Should the recipient decide to not accept the gift certificate, it may be returned to us within 60 days of receipt and we will refund the face value to the person who made the gift (not the recipient).

Please complete the following information and return the order form to us, along with check or money order at the address above. If you prefer to pay by credit card, please use the “Gift Certificate” button on our web site to order a GPH gift certificate through PayPal.

The Gift Certificate will be sent directly to the recipient or to the donor, as you direct (if to the recipient, we will also send a confirmation to the donor).
